ALSF Electronic Motion Form

To be used by individuals or committees who require decisions affected by Chapter By Laws/Standing Rules, Finances/Budgets and/or Policies.
  • Form Guidelines

    Once generated, this electronically signed motion form is sent to the submitter who will forward to all reviewers and approvers under separate emails. This includes the originating committee, Board of Directors and/or general membership. A copy of the form will also be sent to the Recording Secretary. The submitter will need to regenerate if changes are required to the motion before its approved.

    If the motion incorporates, approves or references information contained in one or more separate documents, they must be attached. If your files are in an EXE or DOCX format you will need to resave into a PDF then attach.
