Petition Process 2021-22

This form will be used to submit a nominee name for a board or chapter delegate (primary/alternate) name prior to the slate vote in April.
  • Instructions - Ten percent (10%) or more of chapter members eligible to vote may nominate, by signed petition, an additional nominee for an office on the Board, or for a delegate or alternate by filling out this form and mailing this form along with the written consent of the nominee to the Recording Secretary at least ten (10) days prior to the election meeting on April 28th. No Member shall sign more than (1) nominating petition in a year.

    Submit a nominee name to add to the slate of officers and delegate/alternate to annual meeting to be voted on by members.
