Our organization, along with most others now who work with the public, are requiring all of our members to complete a background check.  It is a simple procedure, and here is what we ask you to do to have yours done:

IF you are a volunteer with Operation School Bell, Operation Art Adventures, The Dental Program, and/or Puppets with Purpose:

  1. Go online to the Salem-Keizer Schools website:  www.salkeiz.k12.or.us
  2. Click on the tab up near the top:  “volunteer”
  3. Click on the highlighted tab in the page “submit a criminal history check”
  4. Fill out the form as instructed
  5. Click submit

You will receive notice that your application has been received via email, probably that same day.

You will receive results of your application via email within about 2 weeks.

Please report that you have submitted this application to the Chair of each of the philanthropies you volunteer your time:  OSB, Dental Program, Puppets with Purpose, Art Adventures.

Chairs will record that you have done so, including the year.

Chairs will send the names of all who have had the check completed to the current Membership Chair.

Additionally, you can check to see when your 5-year check expires, by calling the district at  503-399-3061

If you are a volunteer for Operation Bookshelf and/or R.E.A.D., please provide your full name as it appears on your driver’s license, date of birth, and current address to the chair of your committee.  Chairs will submit the information to library staff, who will ensure the background checks are completed.

There is no charge to you or to ALS-K for doing the checks with the school district or the library.

If you do not volunteer in any of our philanthropies, please submit your full name as it appears on your driver’s license, your date of birth (including year), and current address to Pat Straughan:  membership@als-k.org  or in my box (Membership Committee) at Daue.  All information will be kept strictly confidential.
